Boarding Agreement

I release Palmetto Pet Hospital Inc., its staff, owners, representatives, and agents from any and all liability which I or my pet may suffer including but not limited to injury, sickness, damage or death resulting from participation in overnight boarding and/or grooming.
I represent to the best of my knowledge that my pet is in good health and is current on all required vaccinations (DHPP, Rabies & Bordetella), and is free of fleas, ticks, lice and has not been ill with any known contagious viruses in the last 30 days.
I understand that while my pet is fully vaccinated, immunity varies and there is a small risk that my pet may contract a contagious disease or illness.
I understand that although all dogs are supervised, accidental injuries may occur from playing. I affirm that my pet is sociable and has not harmed or threatened any person or animal. However, if my pet causes injuries or damage to any property, I do understand that I will be responsible for all charges.
BOARDING PRICES: Large run- $41.50/day; Regular run- $35.50/day; Feline- $24.70/day Medication administration- $6.60/day

What if I don’t provide my pet’s food?

Palmetto Pet Hospital is pleased to provide a Hills non-prescription diet for our boarding patients who do not provide their own diet. The charge for kennel supplied meals is $2.20 per meal for pets under 40lbs and $4.40 per meal for 41lbs and greater.

Administering Medication: For non-prescription supplements, we require the original bottle with written instructions

I understand that if my pet needs to be given medication, I will be charged $6.60 per day that medication is administered. For non-prescription supplements, we require the original bottle with written instructions

Pick-up/Drop Off

I agree to pick up my animal on the agreed day of discharge prior to closing time at 6pm. Should I fail to do so I understand that I will be charged late fees in the amount of $5 for every fifteen minutes up to an hour. I understand animals not picked up, one hour after closing will be charged $26.00 in addition to the regular boarding fee and will not be available for pick up until the following business day. I understand that I will be charged additional fees for food and other supplies used for boarding for my animal past my original departure date.
I understand that boarding is charged the full day rate the day I drop off regardless of time. I understand there is no charge if my pet is picked up by 9am. I understand if I pick up after 9am, I will be charged for a full day. I understand that drop off and pick up is permitted only during the hours of Monday through Friday 8:00am-9:00 am in the morning and 3:30-5:30 in the evening.
I agree to pay for all outstanding charges prior to or at the time of pick-up of my pet. If myself, my emergency contact, or any other person I have authorized in writing does not pick up my pet on the agreed upon departure date, I hereby authorize Palmetto Pet Hospital, Inc. to continue to provide boarding and any additional services at my expense.
